When a participant exits in our service, it is useful to complete an Exit form so that

-    we know the reason and address any complaints or advocacy concerns

-    can support the person with referral to other supports

-    can identify any risks that may present to the participant during the transfer to another provider / hospital

-    can let their Support Coordinator know in a formal manner

-    enable Kyeema’s participant exit administrative processes to take place

 

Transitions to or from a provider may entail risks to the participant that need to be identified and communicated between all stakeholders. Clear communication between services and the persons support network helps ensure that critical information about the persons health needs, potential risks and current care are not lost during transition.

It is important to ensure that CCF-06 Exit from Service Form and any supporting documentation is given to the disability support service or hospital during the transition phase.

 

Upon the death of a participant a major incident report is created for the NDIS Commission and a CM note completed on Carelink.

 

Individuals voluntarily leaving the service are required to give notice as per NDIS Guidelines in current price guide and the manager or team leader has discretion to reduce the notice time if appropriate.

 

This helps Kyeema to honour commitments to staff who may need to be re-deployed or have work hours reduced.
